Under "New Patient Information" you will find all of the required paperwork that you will need to complete prior to attending your initial appointment. The information packets may be downloaded, printed and completed at home. If you like it may then be faxed to the office where your initial visit is scheduled.
In "Patient Education" you will find useful educational information regarding various injuries and dysfunctions that are commonly treated with physical therapy.
